New Patient Registration

Please check that you are within our boundary to register for the practice

Registration can be done online or in person.  Please see below steps on your preferred method.

Online

Please visit - https://gp-registration.nhs.uk/N82014 and follow the steps. 

Please fill in your health questionnaire after you have submitted your registration form.

 

In Person 

Please drop in with proof of name and address eg utility bill, passport or driving licence, to confirm that you live in our catchment area. If you do, you will then be able to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be downloaded from the above "Registering Online" tab.

Please note that it can take some time for all of your medical records to be transferred to us.

 

New Patient Health Check

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
