New Patient Registration

Please see below steps on your preferred method.

Registration in person

Complete the paper form to register with a GP.  The paper form can be downloaded here 

Print and fill in the form and return it to the practice you want to register with.

You can also get a copy of this form from the practice.

You may get asked to provide proof of name and address e.g utility bill, passport or driving licence, to confirm that you live in our catchment area. If you do, you will then be able to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. 

Please note that it can take some time for all of your medical records to be transferred to us.

New Patient Health Check

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
