Contacting the Surgery.

If you have a query that relates to the care of you or a named individual then please complete an on-line consultation using Patchs so we can safely and securely handle your request.

We cannot accept orders for repeat medication through this form or through Patchs. Please use one of the on-line services such as Patient Access or the NHS App.

Complaints

Practice Policy is attached.  Complaints are to be put in writing to the Practice Manager.  The completed form will need to sent to the to the manager on this link.

If you are acting on behalf of the patient. Please fill out form. Consent MUST be given by the patient. 

If you are unhappy with the resolve of a complaint, please contact the surgery as soon as possible.  Alternatively you can contact NHS Healthwatch
